Initial Meeting/Treatment Plan

The initial evaluation occurs during our first two sessions. Following the initial evaluation, we will work to discuss my treatment recommendations and your treatment goals.

Once we begin therapy we will usually schedule one 50-60 minute session per week at a mutually agreeable time. Throughout therapy we will continue to assess and revise your treatment goals.


Financial Policies/Fees

Session Fees: All payments are due in full by cash or check at the time of the session unless prior arrangements are made. If using insurance, co-payments and/or deductibles are due at the time of the session.

Late fees: A late cancellation fee of $50.00 will be charged for sessions missed without 24-hour notice (Friday notice for Monday appointments). Once an appointment is scheduled you will be expected to pay for it unless you provide 24 hour notice of cancellation regardless of the reason for cancellation.

Additional Service Fees: Telephone sessions or consultations are billed at the individual session rate on a prorated basis. Preparation of reports or letters will also be billed on a prorated basis at the individual session rate. All professional services related to legal proceedings are billed at $180.00 per hour.

Forms

In order to complete a comprehensive evaluation in a relatively short amount of time, it is helpful for you to complete paperwork prior to your first visit.

If you are a new client, please print and complete the following forms and bring them with you to your first session. If you do not have access to this paperwork, please plan to arrive at least 15 minutes early to your first session so that we can provide you with the paperwork to complete before your first session.

Fees, Cancellation Policy, and Tardiness

The fee for an Intake Session/Diagnostic Interview is $200 and for Individual Counseling is $120 for 45 minutes and $180 for 60 minutes. You will be expected to pay for each session at the time it is held.

You may pay by check or cash. Bills that are 60 days past due may be place in collection. I will inform you before I take this measure so that you will have the opportunity to pay promptly.

Once an appointment is scheduled you will be expected to pay for it unless you provide 24-hours advance notice of cancellation, regardless of the reason for cancellation. If you must cancel and wish to reschedule, I will try to find another time to reschedule the appointment during the same week. I will make every effort to start our sessions on time. Sessions will end 50 minutes after the scheduled appointment time, even if you are late. If (on rare occasion) I have to begin our session late, I will make up the missed time in some mutually agreeable fashion (e.g., by extending the session, if convenient for you).

In addition to the weekly appointments, I may charge the same rate (prorated according to length) for other professional services you need. Other services may include report writing, telephone conversations lasting longer than 15 minutes, and attendance at meetings with other professionals you have authorized.
